A lot of information was included with the newsletter that went home, including several forms which must be returned to the school as soon as possible.
1) Emergency registration card - please complete
2) Two ALERT NOW forms: Early Dismissal Emergency Plan and Emergency Contact Information – Please complete both forms even if it is a repeat of information on the Emergency registration card.
3) Information and application form for free and reduced meals - complete if qualifying
4) School insurance coverage information and application - optional
5) Permission for children to receive fluoride in school - optional, but recommended
6) Permission for over the counter medication - Students may receive this medication at school, only if this form is completed.
7) Prescription Medication Order and Permission Form - complete only if child has prescription medication that must be given at school.
8) Field Trip Permission Forms - please complete
9) Permission to Photograph or Videotape – please complete
10) Permission to have name, address and phone number in a school directory, only to be handed out to Leicester School families — please complete.
11) Lunch menus for the end of August and September
12) Acceptable Computer & Internet Use form —please complete
13) Letter from FOLCS
14) Transportation Handbook to youngest child in family. The handbook contains the bus schedule — please complete.
